How PhoenixCare handles personal information from website visits, contact requests and pilot inquiries.

Overview

PhoenixCare Inc. is a Montréal health-technology company. This policy explains how PhoenixCare collects, uses, discloses, stores, and protects personal information when you visit www.phoenixcare.io, contact us, or submit a pilot or demonstration inquiry through the website.

This website is intended for general company and business information. It does not provide medical advice, diagnosis, or treatment. Do not submit patient records or sensitive medical information through the website contact form or ordinary email.

PhoenixCare’s Designated Privacy Officer is Alex Peav. Privacy questions, complaints, and rights requests may be sent to admin@phoenixcare.io.

Definitions

“Personal information” means information about an identifiable individual, including information that identifies the person directly or indirectly. De-identified information remains personal information when re-identification remains possible. “Anonymized” information means information that irreversibly no longer allows a person to be identified in accordance with applicable law. “Profiling” includes using personal information to assess characteristics such as interests, preferences, behaviour, location, or professional context.

Information we collect

Information you provide

When you email PhoenixCare or use the embedded pilot-request form, we may collect your full name, work email address, organization, organization type, phone number, preferred meeting date and time, pilot-or-demo preference, and the details of your inquiry.

The pilot-request form is provided through Typeform. Do not include patient-identifying or medical information in that form.

Technical and usage information

When you visit the website, PhoenixCare and its website providers may process technical information such as IP address, browser and device information, page and form interactions, timestamps, and security or performance logs.

The current website is hosted through Vercel and embeds Typeform. Typeform may process form-progress and partial-response data and may set cookies or similar identifiers for form delivery, attribution, performance measurement, fraud prevention, and load balancing. An appropriate consent and preference mechanism for non-essential cookies or similar technology is part of this proposed policy and remains subject to verification before it takes effect.

Why we use information

PhoenixCare may use website information to:

  • respond to inquiries and requests;
  • evaluate and coordinate potential pilots, demonstrations, or partnerships;
  • operate, secure, troubleshoot, and improve the website and contact process;
  • prevent misuse, fraud, or security incidents;
  • maintain required business, suppression, privacy-request, and incident records;
  • comply with legal and regulatory obligations.

PhoenixCare does not sell website personal information or use it for cross-context behavioural advertising.

Service providers and processing outside Quebec

PhoenixCare uses service providers to operate the website and respond to inquiries. These currently include Vercel for website hosting and delivery, Typeform for the embedded pilot-request form, and Google Workspace for business email and document collaboration.

These providers may process information in the United States or other jurisdictions. Information processed outside Quebec may be subject to the laws of the jurisdiction where it is processed. PhoenixCare assesses the purpose, sensitivity, safeguards, and destination of cross-border processing and requires appropriate contractual and technical protections.

PhoenixCare may also disclose information when required by law, to investigate or respond to a security incident, to protect legal rights, or as part of a corporate transaction subject to applicable safeguards.

Cookies and similar technologies

The website and embedded Typeform use technologies necessary to deliver and secure the website and form. Typeform may also use attribution or performance-measurement identifiers and may autosave form progress.

The proposed approach is to classify cookies, disable unnecessary partial-response capture, block non-essential technologies until valid consent is obtained where required, and offer a persistent control for changing non-essential-cookie preferences. These controls remain subject to verification before this policy takes effect. You can also restrict or delete cookies using your browser settings, although doing so may affect the form.

Retention

PhoenixCare retains information only for as long as necessary for the stated purpose or as required by law. The following periods are proposed operational limits and remain subject to verification before this policy takes effect:

  • general contact and pilot inquiries, including submitted Typeform responses: up to 24 months after the last meaningful interaction;
  • incomplete or autosaved Typeform responses: only for the shortest period necessary to complete or troubleshoot the inquiry process, subject to a verified Typeform configuration;
  • marketing subscription records, if any: until withdrawal, with limited suppression records retained as necessary to respect the request;
  • website security and server logs: up to 90 days unless required for an active incident or investigation;
  • privacy-rights and complaint records: up to three years after closure;
  • security-incident register records: as required by applicable law and the approved incident-response policy.

At the end of the applicable period, information is securely deleted or anonymized, subject to legal holds and other lawful retention requirements.

Safeguards

PhoenixCare uses administrative, technical, and organizational safeguards appropriate to the sensitivity of the information, including access controls, encryption in transit, logging, vendor review, incident handling, and personnel obligations.

No internet service is risk-free. PhoenixCare investigates suspected confidentiality incidents and provides required notices to regulators and affected individuals when the applicable legal threshold is met.

Your rights and choices

Subject to applicable law, you may request access to or correction of your personal information, withdraw consent, ask how information has been used or disclosed, request deletion when the information is no longer required, or submit a privacy complaint.

PhoenixCare will verify your identity before fulfilling a request. Contact admin@phoenixcare.io. You may also contact the Commission d’accès à l’information du Québec or the Office of the Privacy Commissioner of Canada.

Children

The website and pilot-request form are intended for adults and organizations seeking information about PhoenixCare. PhoenixCare does not knowingly collect personal information from children under 14 through the website without consent from a parent, tutor, or person having parental authority, unless otherwise permitted by law.

Patient and hotline information

The website contact form is not the PhoenixCare COPD Hotline and must not be used to submit patient information. People participating in the PhoenixCare pilot receive a separate COPD Hotline Privacy Notice explaining hotline recordings, consent, clinical information, providers, cross-border processing, retention, and patient rights. The Website Terms of Service govern the marketing website; they do not replace the pilot agreement, clinical consent, or hotline privacy notice.
